Project PUP Photo Policy
When on a visit and a picture is taken showing the face of a person in the facility, we need to have a photo release form filled out and signed per HIPAA requirements. This photo release rule also applies to photos of children/minors needs to be signed by a parent or guardian of the individual.
A picture can be taken from the back or not showing the faces and you do not need this form. It is very important to protect the people in the pictures and not violate the HIPAA act. These rules do not apply to staff personnel if they provide verbal consent. If you follow this procedure you may post pictures.
Patients and others may take pictures of you and your dog at your discretion.
